Measuring total LDL cholesterol volume (LDL-C) fails to capture particle concentration and atherogenic risk in patients with metabolic syndrome or discordance.
1. Advanced Atherogenic Biomarkers
Apolipoprotein B (ApoB) vs. LDL-C
Every atherogenic particle (VLDL, IDL, LDL, Lp(a)) carries exactly one molecule of ApoB. Measuring ApoB provides the exact count of circulating atherogenic particles capable of penetrating the endothelial wall, with optimal targets < 70 mg/dL (< 50 mg/dL for high-risk individuals).
- Lipoprotein(a) [Lp(a)]: Genetically driven pro-thrombotic particle; levels > 50 mg/dL (or > 125 nmol/L) indicate elevated lifetime cardiovascular risk requiring aggressive ApoB lowering.
- High-Sensitivity CRP (hs-CRP): Systemic vascular inflammation marker; target < 0.5 mg/L indicates low arterial inflammatory stress.
